CHC+ Innovations™ is an Outcomes Matter Consulting improvement model. We are a specialist consultancy founded by practitioners, working exclusively with local authorities and system partners across SEND, adult social care and continuing healthcare.
We designed the CHC+ Innovations™ specification, defined what good looks like across every domain, and built the implementation model. Our technology partner HGS UK provides the secure, UK-hosted platform. Between us, we bring sector expertise and technical rigour in equal measure.
